On one of the VPN clients if it is a windows PC, in the CMD box can you type IPCONFIG /ALL as see what is says for DNS ?

If I disconnect pi-hole then both LAN and WAN clients use the router as DNS but do use the seperate DNS correctly.

Last thoughts and for testing purposes, try changing WAN DNS to Pi-hole IP.
and then also if above does not work, change LAN DNS to Router IP

If I change WAN DNS to Pi-hole it stops connection. If I change LAN DNS to router it works but pi-hole stops responding. Just one of those anomolies. I might get an uptodate ASUS router to use latest firmware.

I managed to get hold of a AX-58U ASUS router for a good price, but cannot upgrade with Merlin firmware as it is V2. Not having much look, haha.

That is most unfortunate, hopefully you can return the RT AX-58U as ASUSWRT-Merlin excels with improvements for VPN options
